Made in Photoshop 6, but more than likely able to use in any program.
Note: All of these layers go in order.
1. I started off with this picture. I resized, and cropped to a way that I liked it, then sharpened leaving me with this. 
2. Duplicate the base, then go back to the original base and desaturate. 
Your layers should look something like this now.
3. Now, set the duplicated, coloured layer to screen. You should end up with something like this.
4. Duplicate the base (should be desaturated), drag it to the top, and set it to Color Burn at 14%.
5. Duplicate the base once more, drag it to the top, and set it to Overlay at 69%.
6. Add a new layer, and fill it with #0F1229, and set to Exclusion at 100%.

7. Add a new layer. Use #D42F19 (Shown with white background to see) as a gradient, and make a tiny part of it in the upper left corner. Set it to Soft Light at 100%. Make sure that you erase everything that's covering up her face.

8. Add a new layer. Use #3A4B88 and #974B31 as a gradient. Set it to Lighten at 100%.

9. Add a new layer, and fill it with #292444. Set it to Soft Light at 100%.

10. Add a new layer, and fill it with #3B1673. Set it to Soft Light at 31%.
- 
11. Duplicate the colored base. Set it to Screen at 30%.
- 
12. Add one new layer, and fill it with #926086 Set it to Lighten at 39%.
